From day one, you’ll have a dedicated Acumen Agent as your primary point of contact, backed by around-the-clock call center support and on-demand training, so you can get help where, when, and how you need it.
Our role is to simplify self-direction and keep payments reliable, so you can focus on what matters most—your independence, your goals, and your well-being. Acumen is pleased to offer Family Care Financial Management Services (FMS) through various MCOs across Wisconsin.
For all new enrollments and new hires, please begin by speaking with your Care Team or Care Manager. Once they submit your referral to Acumen, your Acumen Agent will reach out by phone to welcome you and begin your enrollment.
