| Michael T. Davis, Board
of Directors |
Mike Davis, M.Pr.A., C.P.A., manages a
specialized CPA firm based in Salt Lake City, has practiced public accounting for
twenty-one years. He attended the University of Utah and holds a Masters Degree in
Taxation, 1986, a Bachelors Degree in Accounting, 1982, and a Bachelors Degree in
Business Management, 1981, all with honors. He is a member of the American and
Utah Institute(s) of Certified Public Accountants, and publishes for the Utah
Institute. Mike was a volunteer social worker with the Salt Lake County Division
for the Aging during his undergraduate years. After high school, he lived and worked
in Germany for several years, first as an atomic rocket crewman for the U.S. Army,
later as a farmhand, and traveled Europe extensively. He enjoys time with his family
and trains Dalmatians for leisure. He has been a RISE, Inc. Board member since October
2002.

| Theresa Halpine, Board
of Directors |
Theresa Halpine graduated with a B.S. Degree
from the University of Nebraska. She received a Dental Hygiene Degree RDH from the
University of Nebraska Medical Center College of Dentistry and also has an Expanded
Functions Degree in Dental Hygiene. Theresa with her husband owned a dental practice
in Bisbee, Arizona for 23 years where she served on the Human Rights Ethics Committee
for the developmentally disabled for three years. She has recently worked with the
Dental Hygiene Program at the University of Otago in New Zealand for a semester. Her
hobbies include kayaking and tennis. Theresa has been a Board member since 1996.

| Virgil Johnson, Board
of Directors |
Virgil Johnson retired as the Senior Chiropractic
Physician of a five-physician office in Elgin, Illinois. He took his professional training
at the Logan College of Chiropractic in St. Louis, Missouri and also studied at Northwestern
University, Shurtleff College, and North Park University. Virgil and his wife, May, had an
expressive and receptive aphasic daughter. They started a "first of its kind" independent
living facility for multi- handicapped hearing impaired deaf adults, for which they were the
recipients of the "Thomas Jefferson Award". Governor Thompson appointed Virgil to the
Advisory Board of the Illinois Department of Children and Family Services. Virgil was also
a member of the Arizona Governor's Council "Home of Your Own" Board, working to attain
permanent housing for the developmentally disabled. He has also been a RISE, Inc. board
member since 1996 and chairperson since 2001.

| Mary Rose Koszarek,
Board of Directors |
Mary Rose Koszarek has been the Chief
Financial Officer at Northwood Children's Services, which offers residential,
educational and diagnostic services for children with developmental disabilities,
since July 1997. She also serves as Treasurer of Northwood's Board of Directors.
Mary Rose graduated from the University of Minnesota with a BS Degree in 1974, a
BA Degree in 1986 and an MBA in 2003. She was an instructor at Hennepin Technical
College from 1991-1994. She has been a RISE, Inc. Board member since 1998.

| Liz Richey, Board of
Directors |
Liz Richey has been the bookkeeper for
Dave Richey Appraising since 1991 and also worked for RISE, Inc. from 1991-1994
as a secretary in the Corporate Office. She is active in church, community and
school events, and a member of the Val Vista Community Home Owners Association.
Liz loves spending time with her four children ages 3-10. Her hobbies and talents
include sewing, painting and computer work. She has been a Board member since 1994.

| Gerald Nebeker, President
of RISE, Inc., CEO |
Gerald Nebeker founded RISE, Inc.
in 1988. He has served as President and CEO since its inception. Before RISE
was founded, Gerald was a behavioral consultant, program director and school
psychologist. He received a Masters degree in School Psychology from Brigham
Young University in Provo, Utah in 1982. Gerald and his wife Laurel are the
parents of eight children, who are not only their pride and joy, but fill every
waking moment with errands to run. Gerald considers RISE and its employee's part
of his family and holds a deep appreciation for the supports given on behalf of
people with disabilities. He feels he is the luckiest man alive to be associated
with the good people of RISE.

| Michelle Auer, Vice-President
of RISE, Inc., COO |
Michelle Auer joined RISE in August of 1992. She first served as Vice-President and then in July of 1996 she took on the additional role of COO. Before joining RISE, Michelle began in the field of developmental disabilities as a group home and therapeutic foster parent, then continued on to become the coordinator of services for adults with disabilities for Community Psychology and Educational Services. In 2000 Michelle graduated Phi Beta Kappa with her Masters Degree in Management of Disability Service from the University of San Francisco. Michelle is the wife of a terrific guy, Randy, and a mom of three wonderful children. Michelle and Randy have worked together in the field of disabilities their total married life.
